ST4+

This is the final leg of your Emergency Medicine training before you become a Consultant!!

The College of Emergency Medicine is currently piloting run through training to make it easier and to encourage trainees to consider Emergency Medicine as a career (see EM Training Introduction for more information).  

It is still possible to apply for the specialty training (CEM EM Training Recruitment) this is currently being co-ordinated by Health Education Yorkshire and the Humber and you can apply via their website.  Here is some College advice on preparing your portfolio.

Competencies required

Here is a list of the competencies required during your ST4+ years in Emergency Medicine.  The links take you to the College of Emergency Medicine WBPA documents, although most trainees now use the e-portfolio.

During this year you need to complete your FCEM examination and training assessments to be able to apply for and be awarded your certificate for completion of training (CCT).

It is also important that Level 1 competencies (sign-off) in USS are attained during this time.

This a list of the presentations and procedures for Emergency Medicine trainees.

Higher specialty training

During ST4 - the trainee should undertake:

  • Formative assessment (Mini-CEXCbD) in 3 of the major presentations:
    • HMP1 - Anaphylaxis
    • HMP2 - Cardio-respiratory arrest
    • HMP3 - Major trauma
    • HMP4 - Shocked patient
    • HMP5 - Unconscious patient
  • Formative assessment (ACAT-EMMini-CEX or CbD) in 9 acute presentations which must include:
    • HAP3 -  Alcohol and substance abuse
    • HAP5 -  blackout
    • HAP11 - environmental emergencies
    • HAP17 - headache
    • HAP 34 - wound assessment
  • additional assessments of a further 8 acute presentations by
  • Summative assessment of the 3 paediatric major presentations not completed in CT3 (mini-CEX or CbD)
    • PMP1 - anaphylaxis
    • PMP2 - Apnoea, stridor and airway obstruction
    • PMP3 - Cardiorespiratory arrest
    • PMP4 - Major trauma
    • PMP5 - Shocked child
    • PMP6 - Unconscious child
  • Formative assessment (Mini-CEX, CbD or ACAT-EM) of 5 acute paediatric presentations
  • Ultrasound - trainees should complete the theory training and log summary and commence the triggered assessments for

 

During ST5 - the trainee will undertake:

  • Formative assessment (Mini-CEXCbD) in 2 of the  major presentations:

    • HMP1 - Anaphylaxis
    • HMP2 - Cardio-respiratory arrest
    • HMP3 - Major trauma
    • HMP4 - Shocked patient
    • HMP5 - Unconscious patient
  • Formative assessment (ACAT-EMMini-CEX or CbD) in 8 acute presentations which must include:
    • HAP21 - oncological emergencies
    • HAP27 - medical emergencies in pregnancy
  • additional assessments of a further 8 acute presentations by :
  • Formative assessment (ACAT-EM, Mini-CEX or CbD) of 5 acute paediatric presentations 

  • Ultrasound - completion of triggered assessments and sign off

During ST6 there are no assessments as the trainee should focus on the FCEM exam

Common competences

By ST6 the trainee should seek evidence of level 4 competence in all 25 common competences.
